The mission of Methodist Healthcare Ministries of South Texas, Inc. (MHM) is " Serving Humanity to Honor God " by improving the physical, mental and spiritual health of those least served in the Rio Texas Conference area of The United Methodist Church. Social Work Case Manager (Bilingual in Spanish) Dixon Health & Wellness Center San Antonio, Texas Essential Duties: Engages in basic generalist practice that includes interviewing, assessment, planning, intervention, evaluation, case management, information and referral, problem solving, consultation, supportive counseling, education, advocacy and community organization Develops an action plan with goals, based on functioning level, physical/medical condition, support network, psychosocial and financial status based on long and short-term needs Develops and maintains effective working relationships with individual client and/or family Makes referrals to community agencies and conducts follow up activities. Serves as client liaison and advocate Assists clients who are deemed ineligible for Methodist Healthcare Ministries (MHM) services to access alternative community resources Conducts individual and family training in psychosocial management skills Develops and maintains a cumulative record for each client receiving case management services as required by professional licensure standards and MHM policies and procedures. Works as part of an interdisciplinary team to provide integrated behavioral health and primary care services Communicates effectively with clients, MHM staff, and participants in interdisciplinary team Cooperates with and contributes to the development and implementation of a interdisciplinary treatment plan Promotes interactive communication and develops and refines linkages between MHM programs and community health care facilities, social service providers, schools, and other agencies. Cooperates with local medical, mental health and community agencies Maintains documentation of community resources. Participates in activities to develop and maintain quality assurance indicators that evaluate the effectiveness of the case management program. Implements MHM policies and procedures and recommends any needed modifications. Participates in community events during business work week, weekends and evenings as needed Sustains productivity expectations established by supervisor Performs other duties as assigned Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. One year of case management experience required. Certificates and Licenses: Licensed Baccalaureate Social Worker (LBSW) by the Texas State Board of Social Worker Examiners required. Language Skills: Must be able to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Must be able to write reports, business correspondence, and procedure manuals. Must be able to effectively present information and respond to questions from groups of managers, clients, and the general public. Must speak English. Spanish speaking required. Reasoning Ability: Must be able to define problems, collect data, establish facts, and draw valid conclusions. Must be able to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Computer Skills: Must be able to demonstrate working knowledge of the Internet, Outlook, and PowerPoint, Word, Excel and electronic health records. Must be able to efficiently document in electronic health record. Other: Must be able to drive and have access to a car; maintain valid driver's license and auto liability insurance. Work Environment and Physical Demands: The work environment characteristics described here are representative of those a team member encounters while performing the essential functions of this job. While performing the duties of this job, the team member is exposed to an office and residential environment having direct contact with patients and clients. Position may necessitate bending, stooping, twisting, turning, walking, climbing step stools, sitting and standing for periods of time. Must be able to lift and maneuver at least 15 pounds.
