QUALIFICATIONS:
Education: Bachelors of Science in Nursing or completion of
Bachelors in Nursing within one year of accepting position, or
Graduate from a college or university accredited by National League
for Nursing Accrediting Commission (NLNAC), or The Commission on
Collegiate Nursing Education (CCNE)
Certifications: Basic Life Support (BLS)
Licensure: Current, full, active, and unrestricted license to
practice as a Registered Nurse
Experience: Minimum 5 years of full time nursing experience AND 2
years of continuous Disease Management experience.
Requirements: Must be a U.S citizen and have the ability to pass a
background check/security clearance.
ASSIGNED DUTIES: *Please Note: A detailed Statement of Work will be
provided prior to coordination of a formal interview. The Statement
of Work would be specific to the Clinical Department requesting
these services. The HCW shall provide services within the scope of
the clinical privileges granted by the commanding officer, the
applicable duties provided in the basic contract, and the
following:
* Coordinate patient care in collaboration with a wide array of
healthcare professionals. Facilitate the achievement of optimal
outcomes in relation to clinical care, quality and cost
effectiveness.
* Ensure compliance with standards of care and practice in
accordance with all established policies, procedures, and
guidelines used in the MTF.
* Identifies, develops and executes appropriate disease management
activities and interventions such as, but not limited to,
individual and/or group patient contacts and training classes.
* Coordinate patient care in collaboration with a wide array of
healthcare professionals. Strive to improve health outcomes for our
beneficiaries with chronic medical conditions
* The Disease Manager focus will include but not limited to
Diabetes, Asthma, Low Back Pain, Hypertension, Hyperlipidemia, CHF,
COPD and Obesity.
* Management of Chronic Disease per utilization of Evidence-based
Clinical Practice Guidelines (CPG).
* DM proactively reviews CarePoint 4G: MHSPHP for high acuity
patients. High acuity patients include but are not limited to high
utilizers, chronic disease in focused areas, admission follow up
for chronic disease, high ACG RUB/IBI scores, or elevated lab
values (e.g. hemoglobin A1Cs or LDLs).
* The DM identifies patients who require interdisciplinary services
and makes appropriate referrals as needed.
* Communicate and collaborate with a diverse group of people for
the purpose of informing the healthcare team of plans/actions, for
teaching/education to benefit the patient/family and
organization.
* Perform other nursing services required.
SPECIFIC DISEASE MANAGEMENT DUTIES:
* Conduct research in support of improved practice and patient
outcomes.
* The DM nurses empower the patient with self-management tools and
provide education about disease.
* The DM nurses ascertain the patients willingness to modify
current practices and behaviors to avoid long term complications
associated with chronic illnesses.
* The DM nurses follow-up to ensure patients understanding of their
chronic medical condition and provide additional education and/or
resources as needed.
* The DM will ensure patient has proper education on use of
medications they are taking.
SCHEDULING AND STAFFING:
Duty Hours/Shifts: HCW hours of performance are 8 or 10 hour shifts
between 0630 1700. Not to exceed 40 hours per week or 80 hours per
two weeks.
On-Call Hours: N/A
Overage Hours: N/A
Place of Performance: Medical Treatment Facility at SAMHS Market,
Joint Base San Antonio and Bulverde Clinic.
