Job SummaryWe are seeking a skilled Registered Nurse Transitional Care Manager to join our team at Baptist Health System. As a key member of our healthcare team, you will play a vital role in facilitating discharge planning and post-acute care services for patients with complex medical conditions.Key Responsibilities

• Collaborate with the Case Management department directors and senior leadership to develop and implement effective care plans.
• Coordinate care and services for selected patient populations, focusing on patients with Congestive Heart Failure, Acute Myocardial Infarction, and Pneumonia.
• Design and implement plans of care to prevent frequent acute hospital or emergency department use, readmissions, and health status decline.
• Complete established competencies for the position within a designated introductory period.Requirements
  • Required: Bachelor of Science in Nursing (BSN) if Magnet hospital.
  • Preferred: Bachelor of Science in Nursing (BSN).
  • Required: 3 years acute nursing experience, healthcare provider or healthcare company experience.
  • Preferred: 2 years experience in Utilization Review, Discharge Planning, and Medical Case Management in a hospital setting.
  • Strong analytical and organizational skills.
  • Working knowledge and ability to apply professional standards of practice in the work environment.
  • Knowledge of state regulatory, federal regulatory, and managed care requirements.Certifications
    • Required:
      • Registered Nurse/RN License.
      • BLS-Obtained through approved American Heart Association Training Center or the Military Training Network.Tenet complies with federal, state, and/or local laws regarding mandatory vaccination of its workforce.
