Registered Nurse -Intermediate (Primary Care Center)Job Summary:

Under general supervision, provides direct nursing care to patients in a healthcare facility. Responsible for assisting the clinicians in the Primary Care Center Virtual Clinic Caf-- that supports the Primary Care Center clinics.

Job Duties:

1. Provides nursing care to patients in a virtual environment via telephone and/or electronic communication for the Primary Care Center Virtual Caf-- clinic using the Primary Care Center policies and procedures.

2. Assists assigned medical providers and mid-levels including but not limited to: providing clinicians interpretations of results, consulting with clinicians and communicating with patients their care plans.

3. Develops protocols and procedure manual for the Primary Care Center Clinic Caf--

4. Calls-in prescriptions and refills for patients.

5. Triages patient calls and works closely with PCC clinical staff and clinicians.

6. Follows-up on diagnostic tests and labs, and ensures they are reported to the medical provider.

7. Documents all patient care interactions in the electronic medical record.

8. Performs all other duties as assigned.

Education:

Associate's degree is required.

Experience:

Three (3) years of professional nursing experience is required.

Additional Licenses and Certifications:

Current Texas RN license; CPR required.
