The Health and Social Services Manager will lead a care management team while overseeing day-to-day clinical operations and health plan clinical initiatives to support achieving quality and affordability targets. - The HSS Managers will support a team within different regions in Southern Indiana. - Responsibilities will include process improvement, execution of clinical decision processes, and team management, to include performance management and implementation of program initiatives. -

Primary Responsibilities: -

• 
  
• Selects, manages, develops, mentors and supports staff in designated department or region
  
• Develops clear goals and objectives for performance management and effectively communicates expectations, and holds the team accountable for results
  
• In order to meet the unique needs of our members, have an intimate understanding of the contractual requirements
  
• Identify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirements
  
• Conducts monthly audits of staff and participates in all other required audits and business monitoring
  
• Ensures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherence
  
• Participates in training and coaching of direct reports as needed
  
• Collaborates across Optum and UHG and interacted with Medical Directors, Senior Leaders, Quality, and other stakeholders

  Required Qualifications: -
  
  
  
  • 
    
  • Active and unrestricted RN license in the state of Indiana or Master's level social worker
    
  • 5+ years of experience in providing care coordination to older adults
    
  • 4+ years of experience working within the community health setting in a health care role
    
  • 3+ years management experience
    
  • Experience in LTSS or working with the program population
    
  • Experience facilitating and working in coordination with an Interdisciplinary Team
    
  • Experience coaching or mentoring staff
    
  • Intermediate level of experience with Microsoft Word and Excel
    
  • Proven ability to navigate a TEAMs environment
    
  • Resident of Indiana
    
    
    Preferred Qualifications:
    
    • 
      
    • Case Management experience
      
    • Experience with LTSS or Waivers
      
    • Background in managed care
