POSITION SUMMARY/RESPONSIBILITIES
Supports the Director in supervision of the team to facilitate
positive health outcomes for our membership and compliance with
quality and regulatory requirements. Works with all staff within
the team to monitor and prioritize workloads, as well as formulate
and communicate program planning that efficiently utilize health
care services in the safest, most cost-effective manner. Monitors
and analyzes routine, ad hoc reports, case load, and care
coordination management efforts for the team. Assists Community
First Health Plan, Inc. (Community First) members regain optimum
health or improved functional capacity by managing teams which
ensure that members have access to all the health care services
they need in the most efficient and effective manner possible.
Responsibilities include but are not limited to: -Assists Director
in developing, planning, and organizing health promotion and
disease management programs and in defining implementation
priorities for various Population Health Management (PHM) projects
to ensure the successful attainment of organizational goals and
outcomes. Collaborates with internal and external departments for
purposes of data collection, analysis, project management, and
implementation. - Creates customized reports based on analysis of
data for dissemination and participates in efforts to improve
operational performance. Researches and utilizes industry, state,
and national benchmarks. Assists Director in reviewing program
effectiveness and metric results and implementing improvement
initiatives
EDUCATION/EXPERIENCE
Bachelor's degree in nursing is required. A master's degree in
public health (MPH) or a master's in health care or business is
preferred. Three (3) years' experience in project management and/or
coordination preferred. Knowledge of program administration,
evaluation, and financial management is preferred. Strong clinical
knowledge and understanding of evidence-based clinical guidelines
related to chronic disease management required. Excellent
leadership and communication skills. - Bilingual preferred
(Spanish)
LICENSURE/CERTIFICATION
Current licensure as a Registered Nurse with the Texas State Board
of Nurse Examiners is required.
