together. /n /n The Licensed Practical/Vocational Nurse is
responsible for providing direct patient care under the supervision
of a registered nurse. Responsibilities include following the plan
of care, providing treatments, and working collaboratively with the
members of the team to help meet positive patient care outcomes. /n
/n Primary Responsibilities: /n /n - Provides direct patient care
as defined in the state Nurse Practice Act and agency's policies /n
- Implements plan of care initiated by the registered nurse /n -
Provides accurate and timely documentation consistent with the plan
of care /n - Assesses and provides patient and family/caregiver
education and information pertinent to diagnosis and plan of care
/n - Participates in the coordination of home health services,
appropriately reporting the identified needs for other disciplines
(HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse
and/or Clinical Supervisor /n - Follows all infection control
standard precautions and safety guidelines/standards as per agency
policy /n - Uses equipment and supplies effectively and efficiently
/n - Participates in personal and professional growth and
development /n - Adheres to the Agency's Standard Operating
Procedures as it relates to the submission of the documentation /n
- Performs all other related nursing duties as assigned /n -
Communication /n /n - Prepares clinical notes and updates the
primary physician when necessary /n - Communicates with the
physician regarding the patient's needs and reports any changes in
the patient's condition; obtains/receives physician's orders as
required /n - Coordinates services and schedules with the Clinical
Team Manager (CTM) and Clinical Team /n - Coordinator
(CTC/Scheduler) to include recommendations for additional home
health care services for patients by the end of the worked day /n
/n /n - Additional Duties /n /n - Participates in on-call duties
within the on-call rotation schedule, to include weekends as
assigned /n - Demonstrates knowledge and observance of Patient
Rights and Notice of Privacy Practices /n - Ensures that
arrangements for equipment and other necessary items and services
are available /n - Maintains a daily patient case load and point of
care documentation levels, as per Agency standards /n - Ability to
work flexible schedule to meet patient's needs /n - Demonstrates
personal responsibility with regard to attendance and punctuality
/n - Maintains privacy and confidentiality with regard to all
patient, staff and agency information /n - Demonstrates
flexibility, enthusiasm and willingness to cooperate while working
with others or in place of others, as necessary /n - Expresses
verbal and written communication in a clear, positive and
cooperative manner /n - Ability to work flexible schedule to meet
patient's needs /n - Promotes the agency image by adhering to the

Required Qualifications: /n /n - High school diploma or equivalent
/n - Graduate of an accredited practical nurse or vocational
nursing program /n - Current unrestricted license in the state of
Texas as a practical nurse or licensed vocational nurse /n -
Current CPR- BLS certification OR ability to obtain prior to start
of employment /n - 2 years of clinical experience preferably in a
community home health or medical/surgical setting /n - Thorough
knowledge of current nursing practice, professional standards of
care, and state and federal regulations regarding home care /n -
Proven ability to comply with accepted professional standards and
practices /n - Proven ability to endure prolonged or considerable
walking or standing; lift position or transfer patients i proximate
location; lift supplies and equipment; perform reaching, stooping,
bending, kneeling or crouching. Visual acuity and hearing,
functional or corrected, to perform required nursing skills /n -
Valid Texas driver's license and maintain personal auto insurance
coverage in accordance with organization requirements /n - This
position requires 6 - 8 week in office training /n /n /n Preferred
Qualifications: /n /n - Experience working with electronic medical
record application /n - Proven ability to prioritize and
communicate objectives clearly /n - Proven ability to interact
productively with individuals and with multidisciplinary teams /n -
Demonstrated excellent observation, verbal and written
communication skills, problem solving, basic math skills, and
nursing skills per competency checklist /n - Proven evidence of
independent nursing practice in delivering nursing care /n - Proven
basic computer skills to include Microsoft Word, Outlook and Other
email systems /n - Proven excellent organizational and
prioritization skills /n /n /n Physical & Mental Requirements: /n
/n - Ability to lift up to 50 pounds (must be able to lift and
transfer patients from one location to the other, if necessary) /n
- Ability to push or pull heavy objects using up to 50 pounds of
force /n - Ability to sit for extended periods of time /n - Ability
to stand for extended periods of time /n - Ability to use fine
motor skills to operate office equipment and/or machinery /n -
Ability to properly drive and operate a personal/company vehicle /n
- Ability to receive and comprehend instructions verbally and/or in
writing /n - Ability to use logical reasoning for simple and
complex situations /n /n /n Work Environment (patient's home): /n

all times. /n /n /n The hourly range for this role is $19.86 to
$38.85 per hour based on full-time employment. Pay is based on
several factors including but not limited to local labor markets,
