POSITION SUMMARY/RESPONSIBILITIES The Executive Director of Care Coordination will support the engagement and success of care transition programs, outcomes, and policies; provide oversight and direction for the care coordination services (case managers, social workers and discharge expediters) across the continuum of care for the Health System; and serve as an internal consultant responsible for the integration of the Lean principles within the organization’s culture. The Executive Director will prioritize, direct and facilitate the system wide activities and education to ensure a successful integration of Lean. Directs the care coordination activities at the hospital and the ambulatory care facilities. Collaborates with the physicians and nursing staff to ensure safe discharge planning and a timely and effective discharge of the patient. EDUCATION/EXPERIENCE Master’s degree in Health Care Administration or related field is required. Bachelors of Science in Nursing, Master’s degree in Nursing, preferred. Working knowledge of insurance and third party payer processes. Knowledge of the CMS Conditions of Participation and additional regulatory and accreditation standards. Minimum of five years management experience in an acute care hospital. Knowledge of personnel, administrative and financial procedures and practices, as well as principles, techniques, terminology and procedures of sound management is required. LICENSURE/CERTIFICATION Certification in Accredited Case Manager (ACM) is preferred.
