Job Description:
HCA Healthcare is seeking a dedicated Clinical Transitional Care
Registered Nurse to join our team, focused on managing patient care
and ensuring successful transitions between healthcare settings. In
this role, you will collaborate with other healthcare professionals
to plan, manage, and evaluate patient care, with a special focus on
transitions from the hospital to home or other care settings. This
position requires a strong commitment to enhancing patient
outcomes, streamlining care transitions, and supporting patients
and their families through complex healthcare processes. Assess
patient needs and develop care plans for transitions. Coordinate
with healthcare professionals to ensure seamless transitions.
Educate patients and families about care processes and follow-up
needs. Monitor patient outcomes and adjust care plans as necessary.
Participate in multidisciplinary team meetings to discuss patient
care. Ensure compliance with healthcare regulations and standards.
Strong clinical skills in various healthcare settings Excellent
communication and interpersonal skills Proven ability to coordinate
care across multiple settings Knowledge of healthcare regulations
and standards Capability to work independently and make critical
decisions Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Valid Registered Nurse (RN) license in the state of
practice Basic Life Support (BLS) certification Preferred:
Certified in Care Coordination and Transition Management
(CCCTM)
