Description
The Field Supervisor is supervised by the Branch Manager, Regional Director, and Executive Director. The Field Supervisor is responsible for the management and coordination of the patient services within the agency and oversees the Personal Attendant (PCA). Primary responsibilities include in-person supervisory visits to assess and document that the services delivery plan is adequate, client continues to need the services, needs a services delivery plan change, ensures attendant is competent and is providing the authorized tasks. Document supervisory visits and maintain client's documentation in the client's file. Combine a supervisory visit and a new attendant orientation. Work schedule may vary as needed to appropriately manage the services.
Essential Job Functions
Attends work on a regular and predictable schedule in accordance with agency leave policy and preforms other duties as assigned. 1. Ensures health and safety of clients/staff. 2. Communicating with State Case Workers and MCO Service Coordinators regarding service breaks and /or client's issues in a timely manner. 3. Manage the PCA staff, including, but not limited to, staff evaluations, monitoring and scheduling required staff training. 4. Staffing PCA's for clients. 5. Completes required forms and documents in accordance with the company policy and state and/or federal regulations. 6. Collects and assembles project for weekly/monthly reporting. 7. Completes required visits and submits documentation to be entered in a timely manner basis. 8. Preforms quality assurance for client charts. 9. Has line authority over all staff in the agency: monitors PCA schedules, leave approval, and follows the agency's procedures regarding overtime and leave. 10. Preforms special assignments at the request of the Regional Director and Executive Director. 11. May serve as acting Regional Director as assigned.

Ability to coordinate with other staff, departments, officials, agencies, organizations, and the public. Ability to prepare concise reports, summaries, documents, and other.

Ability to coordinate and collaborate multiple agencies and lead contacts. Ability to adhere to strict confidentiality guidelines. Ability to provide excellent customer service and strong cultural competency. Ability to demonstrate commitment to the to the agency's mission and workplace/service values. Ability to lift 50 pounds. Ability to stand/walk on hard floors for extended period.
Requirements

  • License: Must have current driver's license or reliable transportation to travel to assignments.
  • Experience: If at least 18 years of age, have proof of competency through education and/or experience or demonstrate competency to perform tasks as assigned. If under age 18, must successfully demonstrate competency to perform tasks assigned.
  • Skills: Must be able to follow written and verbal instructions and be competent to perform tasks assigned by Supervisor Demonstrates interest in the welfare of those who are elderly and/or disabled.
  • Background Checks: Must agree to and pass a criminal history check and an Employee Misconduct Registry check.
  • Environmental and Working Conditions: Works in patient's/client's residence in various conditions; possible exposure to blood, body fluids and infectious diseases; must have the ability to work a flexible schedule and travel locally; some exposure to unpleasant weather.
