    We are growing and adding positions to our team! The job duties for the position of Field Supervisor are listed below.

    A Field Supervisor will perform home visits and is responsible for communicating the clients care plan program rules and requirements to clients, caregivers, families and interested parties.

    Acara Healthcare is looking for a Field Supervisor who will work closely with the Care Coordinator, clients and caregivers within assigned caseload to ensure clients are receiving approved services on an on-going basis according to the Plan of Care.

    Duties and Responsibilities:
    • Knowledge of Personal Care Services (Medicaid) and state requirements,
    • Comply with client's rights and responsibilities.
    • Ensures client and employee privacy and confidentiality pursuant to HIPAA Privacy Rules.
    • Complete initial evaluations and home assessments for clients.
    • Monitoring hospitalizations and/or all suspensions, holds and service interruptions.
    • Assessing and confirming service plan tasks meet clients' needs.
    • Ensuring tasks are not duplicated with any other services received by the client.
    • Ensure Coordination of Care with required forms and communication
    • Scheduling according to service plans, authorized hours and required days per week.
    • Conducts Attendant Orientations with Caregivers and complies with client's caregiver choice if choice meets employee hiring requirements.
    • Conducts client orientations to ensure care plan is being adhered according to the rules and regulations specified by the State.
    • Supervises caregivers ensuring client's individualized service care plan meets requirements, and client's needs are met.
    • Provides training to caregivers with Electronic Visit Verification procedures to ensure that the caregiver understands the processes with clocking in and out per scheduled hours.
    • Field Supervisor will occasionally assist with servicing the clients to avoid service interruptions and ensure that client's needs are being met.
    • Provide oversight to caregivers who are providing PHC services to clients.
    • Maintains daily contact with office support staff.
    • Reports any concerns, issues or change in client's condition to appropriate Agency staff.
    • Routinely perform Personal Home Care Supervisory Visits.
    • Orients caregivers to new assignments and clients' needs according to the care plan.
    • Explains in detail all tasks and schedules with both the client and caregiver.
    • Determines attendant's competency to perform tasks.
    • Provides training and/ or demonstration of tasks to caregivers.
    • Resolves problems and/ or misunderstandings between client and caregivers.
    • Evaluates caregivers job performance.
    • Completes required work documentation, processes and procedures in a timely manner.
    • Completes any and all necessary documentation for all supervisory visits performed with the clients and caregivers.
    • Communicates with respective case workers when and as needed.
    • Documents information about client services, issues and resolutions while maintaining completed compliant forms signed by client.
    • Participates in community activities and education opportunities.
    • Assists the agency with surveys conducted by the State.
    • Maintain high visibility and availability with office staff as well as with clients and caregivers.
    • Maintain confidentiality of all client, physician, and company propriety information.

      Working Conditions:
      • Works in office areas as well as in client homes and in assisted and independent living facilities. Interacts with clients, family members, staff, visitors, government agencies/personnel, etc., under all conditions/circumstances.
      • Is subject to hostile and emotionally upset clients, family members, staff, visitors, etc. under all conditions/circumstances.
      • May be exposed to infectious waste, diseases, conditions, blood or blood borne pathogens and will require OSHA training.
      • Communicates with medical staff, nursing staff, and other unit supervisors.
      • Maintains a liaison with residents, their families, support staff, etc. to assure that the residents' needs are continually met. Education:
        • High School Diploma or equivalent required.
        • Two (2) years of college or may substitute each year of college with supervisory experience at a healthcare facility, home care agency or community agency. Licenses:
          • Valid State Driver's License with good driving record. Language skills:
            • Ability to read and write English.
            • Basic computer skills preferred.
            • Spanish Speaking preferred Job Type: Full-time
