Job Description Job Description JOB SUMMARY: The Group
Facilitator II/Parent Facilitator II conducts outreach and
recruitment for program participation; facilitates multi-component
intervention, prevention, and educational programs; and refers
clients for counseling services and other community resources,
while ensuring that services provided are in alignment with the
Social Determinants of Health through a trauma informed care lens
as defined by the agency. ORGANIZATIONAL RELATIONSHIPS: Reports to:
Service Coordinator or as designated Directs: Monitor on-site
volunteers; liaison with and provide leadership to personnel and
representatives of collaborating agencies, local governments,
school districts, and community residents. Other: Works
cooperatively with all staff to accomplish the mission, goals,
objectives and performance measures of the program and agency
ESSENTIAL PERFORMANCE REQUIREMENTS: Required Competencies/Skills:
Healthcare and Human Services Environments: Ability to explain
issues and advancements in the healthcare and human services
industries. Achievement Orientation: A concern for surpassing a
standard of excellence. The standard may be one's own past

challenging goals, or something that has not been done previously.
Community Orientation: Ability to align one's own and the
organization's priorities and assess and address community needs in

professional practices, as well as stimulation social

people as well as to accurately hear and understand the unspoken or
partly expressed thoughts, feelings, and concerns of others. It
measures increasing complexity and depth of understanding of others
and includes cross-cultural sensitivity through a trauma-informed
care lens. Agency Functions: Community and Customer Service -
Increase client access, enrollment, and satisfaction of program
resources by creating wraparound services, recruitment, and
surveying strategies. Internal Processes - Use agency and program
data to track clients through different agency programs to gauge
resiliency and outcomes of participating families. Learning and
Growth - Strengthen staff and client well-being by addressing the
social determinants of health: Economic Stability, Education,
Health and Healthcare, Neighborhood supports, Social and Community
Engagement. QUALIFICATIONS: To perform this job satisfactorily, an

functions listed above. Education/Training/Experience: Bachelor’s
degree in education, health, social work, or other related human
service field, plus two (2) years of related work experience, with
some background in developing and presenting prevention and
education workshops or programs is desirable, case management or
social service delivery; Associate’s degree in education, health,
social work, or other related human service field plus four (4)
years of related work experience with some background in developing
and presenting prevention and education workshops or programs is
desirable, case management or social service delivery; High School
Diploma or equivalent plus four (7) years of related work
experience, with some background in developing and presenting
prevention and education workshops or programs is desirable, case
management or social service delivery. Experience in understanding
cultural differences and is sensitive to the implications of
culture for service delivery. Fluent in English and Spanish (read,
write, and speak) desirable. Staff will be required to submit proof
of their vaccination status or may request to be exempt from
vaccination due to a religious and/or medical accommodation.
Certificate or license, as applicable, by the appropriate State
agency as a qualified practitioner. State of Texas vehicle
operator’s license and proof of liability insurance PHYSICAL

agency’s hours of operation are extended and non-traditional, and
may vary based on office location and/or stakeholder requirements.
Employees are asked to work the schedule that is best suited to the
achievement of deliverables and stakeholder requirements based on
the agency’s posted business hours, or as required by stakeholder
and contractual requirements. Employees who work in an exempt
position are paid on a salaried basis and are not entitled to
overtime pay. The amount of salary is intended to compensate the
