Varco Hospice is seeking a compassionate and hospice-experienced
Social Worker (MSW) to join our interdisciplinary team. This is a
PRN position only supporting patients and families throughout the
end-of-life journey. Responsibilities Provide medical social
services in accordance with the patient’s Plan of Care Complete
psychosocial assessments within 5 days of hospice admission Support
patients and families with end-of-life planning, coping, and
advocacy Assist with community resources, advance planning, and
discharge planning as needed Observe, document, and report changes
in patient and family psychosocial status Participate in IDG and
QAPI meetings Maintain accurate, timely clinical documentation
Serve as a resource and consultant to the interdisciplinary team
Requirements Active Texas Social Work license (MSW required; BSW
considered per hospice regulations) Minimum of 2 years hospice
social work experience required One year of healthcare social work
experience Current TB or chest X-ray and CPR certification Valid
driver’s license and willingness to travel to patient homes and
facilities Strong communication and documentation skills Position
Details PRN only Flexible scheduling Hospice field-based role
